Support for adults with chronic conditions

If you experience limitations as a result of a chronic condition or have a life limiting illness and wish to receive care at home, support is available.

We work together with a range of partners to co-design and deliver innovative care programs. Our locally based health and community partners include, for example WA Country Health Service, WA Primary Health Alliance and Asthma WA.

Our capability and expertise enables us to provide a diverse offering of programs across different chronic conditions.

Health Navigator Program

Our Health Navigator Coordinators work with people who have chronic health conditions to help you identify health concerns and problems, and develop a personalised plan to improve your health and wellbeing. This might include linking you with other services, providing you with information about your conditions or supporting you to make a change in your life. 

We work alongside health care professionals you are already seeing, such as your GP. You can choose to access this service over the phone or by using video conferencing.

How do I access these programs?

Support for adults with chronic conditions

We understand that every person has different needs and different wishes and that's how our team of caring experts can help.

If you experience limitations as a result of a chronic condition or have a life limiting illness and wish to receive end of life care at home, support is available.

Government-subsidised support may be available if you:

  • live in metropolitan Adelaide
  • are over 18 years old and under 65 years old (or under 50 years for Aboriginal and Torres Strait Islander persons)
  • face significant functional limitations caused by your health
  • are not eligible for other programs such as the National Disability Insurance Scheme.

You will receive care that is completely personalised to you and your situation.

Allied health and therapy services

Allied Health and Therapy services can be accessed individually or as an integrated team to restore, improve or maintain your safety, independence and wellbeing.

Restorative services

These short-term, home based services, aim to improve and strengthen your confidence to move around, connect with your community and assist you to relearn activities that help you regain your mobility and independence.

This can be any combination of the services listed here, which aim to support you to remain living at home.

Respite and carer support

Our staff can provide support to you, whilst your carer has some time out to do other things of their choice.

Personal care

We can help you with daily self-care tasks such as showering and dressing to support you at home.

Domestic assistance

We can also help with cleaning and other household tasks. We can also help with situations of squalor and hoarding.

Social support

Support is available to assist you with your shopping or support you to attend other community activities.

Equipment and home modifications

We can assist you to access equipment and home modifications through the Department of Human Services, Equipment Program, delivered through the Domiciliary Equipment Service.

Equipment can include items such as a shower chair or walking frame to help you be active and manage daily tasks.

Home modifications might include a grab rail or a ramp to improve access and safety in your home.

Chronic Obstructive Pulmonary Disease 

For people with chronic obstructive pulmonary disease (COPD), this program helps you to get back on your feet after a stay in hospital by working towards your goals, as well as providing support and education.

Our care team will complete a holistic assessment with you to ensure you have appropriate referrals, prescriptions and the supports you need for you respiratory condition and any other health conditions you may have.

Following this, Asthma WA’s Respiratory Health Team will work with you to provide COPD education and support for a period of three to six months.

This program is supported by WA Primary Health Alliance.

Integrum Aged Care+

Our Integrum Aged Care+ is an innovative model of health care designed to help you proactively manage chronic conditions and improve your everyday life.
