To do this our care team can assess your mobility around the home and coordinate the installation of simple but helpful modifications to help you stay active and safe, such as grab rails and ramps for example.
Our Occupational Therapy team can help with a range of things such as:
- Assessing your home for falls risks
- Assisting you to put in place fall prevention strategies such as regular exercise, lighting improvements, medication reviews, and small home modifications
- Providing fixed or portable mobility aids
- Seating and wheelchairs
- Support and strategies if you or your loved one are living with dementia.
Our care team will take time to get to know you, and discuss your needs, focusing on your safety and wellbeing. Together we can develop an in home care plan tailored your needs to enable you to stay safe, independent, and confident on your feet.

Who can access our services?
Our equipment and home modification services are suitable for people who may need a little support with everyday tasks due to age, impaired mobility, illness or disability.
Fees and pricing
Services can be self-funded for any person of any age. If you or your loved one is aged 65 years and over, or are an Aboriginal Torres Strait Islander over 50 years, you may be eligible for government funding for aged care services through the Commonwealth Home Support Programme (CHSP) or through a Home Care Package.
As an RDNS Silverchain client, you will need to pay a small contribution fee for our services. These fees help us to deliver more services to more people in South Australia.
