What can be treated?

We can treat a wide range of conditions that would otherwise require a visit to the hospital. This includes, but is not limited to:

  • Pneumonia and severe chest infections
  • Urinary tract infections (UTI)
  • Bone infections (osteomyelitis)

We find that home is a more comfortable, less stressful environment and can improve the quality of life for you and your family and carers. Care is provided both in your home and in our community clinics, depending on the treatment required. Home and clinic visits may be scheduled between 7 am and 9 pm, with phone-based nursing support available 24 hours a day, seven days a week.

What treatments are offered?

We offer a wide range of treatments that would otherwise require a visit to the hospital. These include, but are not limited to:

  • Intravenous antibiotics
  • Intravenous rehydration therapy
  • Iron infusions (this treatment will require a visit to one of our clinics)
  • Management of blood thinners
  • Wound care
  • Catheter management
  • Gastrostomy (PEG) tube management.

Priority Response Assessment

Our Priority Response Assessment (PRA) is a unique assessment service that provides advanced clinical assessments in your home or residential care facility. This is designed to help you avoid a hospital or emergency department visit.

This home visit service is available between 7 am and 9 pm, seven days a week. If your request for assessment is appropriate for a home visit, our PRA nurses will respond to your call with an appropriate time for a home visit.

Phone-based nursing support is also available 24 hours a day, seven days a week.

Who is eligible?

Along with other criteria, this service is designed for people over the age of 13 who have a condition that would otherwise require them to be admitted to a hospital and require daily medical attention. Get in touch to find out if you are eligible.

What is the cost?

This service is funded by the Western Australian government so will generally be free for most clients. You will be notified of any required contributions when your referral is received.
