Our qualified team can help you get back to your normal life at home. We can also reduce the need for ongoing support or to return to hospital.
We start with a comprehensive assessment in your home, where we will look at the support you need and the goals you’d like to work towards.
We can then support you with:
- A range of services to help you live as independently as possible, which includes personal care and allied health services.
- Strategies to improve your confidence and reduce anxiety when you return home, such as breaking down large tasks into smaller steps.
- Exercises and therapy assistance support to prevent or delay your health from getting worse.
- Help to reduce your risk of falling and to age well.
Our care can be delivered by a multi-disciplinary team of professionals including occupational therapists, physiotherapists, therapy assistants, and care aides. The program runs for up to six weeks and can be provided in your home or in the community, such as a park.
We work with you, your hospital health network, and health professionals to make sure you receive the best care.

Hospital Discharge Support
Our Hospital Discharge Support program is available in Perth, and South West and Great Southern regions of Western Australia (WA).
Private Care
Hospital transition services including Allied Health and Personal Care are available privately in Perth and some regional areas in WA.

All people aged 18 and over living in Perth, in need of personal care services, and with reablement goals may be eligible for this program.
You will need to be referred by your hospital clinician while you are in hospital.
Private Care
All people aged 18 and over are eligible for private services.
This program is funded by the Department of Health WA and is free for clients with a referral.
