Our personalised Transition Care Package is a short term, intensive program designed to help you feel completely confident returning to your home knowing that our caring expert team are on hand to assist you get back on track.

Transition Care Packages are for you if you are currently in hospital but feel that you could benefit from receiving assistance and care at home for a short period of time.

What can the packages include? 

Our Transition Care Packages can include a range of services that are specifically designed to help you get back to living independently after your hospital stay. Our team will work with you to understand your needs, and bring together a range of services to help you achieve your goals.

How can I access Transition Care Packages?

The best thing to do is to talk to hospital staff while you’re still in hospital to see if Transition Care is suitable for you. If it is, they can refer you for an assessment, or you can organise an Aged Care Assessment Team (ACAT) assessment by contacting our team or My Aged Care. You are no longer eligible for this program once you’ve left hospital.
