Improving palliative care outcomes and client and carer experience

Silverchain has over 40 years experience delivering community palliative care. Palliative care aims to prevent and relieve suffering and improve the quality of life of people facing problems associated with life limiting illness. Earlier access to palliative care improves quality of life, enhances patient and family experience, and reduces the burden on acute care. Silverchain is engaged in several impactful projects designed to improve outcomes for people receiving palliative care, as well as their family carers. Our key projects include:

  • Conducting an epidemiological analysis of palliative care patient datasets.
  • Developing a palliative care patient and carer portal.
  • Developing a palliative care aged care outcomes program.
  • Exploring palliative care patient trajectories and outcomes through a linked data asset.
  • Caring for the carers: Examining the impact on informal carers of palliative care patients.

Wound research

As internationally recognised leaders in wound care, we are dedicated to a culture of continuous improvement in wound healing. We empower our clinicians to seek, develop, and embrace innovative, holistic, and patient centred wound care practices. Silverchain's wound care research encompasses a range of critical projects designed to enhance patient outcomes and advance the field. Our key projects include:

  • Wound infection study to validate the effectiveness of the International Wound Infection. Wound Infection Continuum signs and symptoms used by nurses to assess and treat wounds, ultimately improving diagnostic accuracy and patient outcomes.
  • Preventing pressure injuries in cancer patients to determine if pressure injuries are avoidable in patients receiving community palliative care programs.
  • Leading research to determine the impact of other health conditions on wound healing in the community.
