The Assistive Technology and Home Modifications (AT-HM) Scheme provides funding for equipment and products that help you with everyday activities. It also funds home modifications to improve safety, accessibility, and independence in your home.

Our Allied Health professionals can recommend suitable home modifications, equipment and products to meet your needs. If you are eligible for AT-HM, you have up to 12 months to use your funding, which is in addition to your ongoing Support at Home funding.  

If you have specific needs like a permanent disability, you may also be eligible for additional funding for a longer period. 

Assistive technology products and equipment you can purchase
  • Mobility equipment like walking sticks, frames and wheelchairs.
  • Toileting aids such as bedpans and commode chairs.
  • Washing, bathing and showering support like non slip bathmats, shower chairs, or handrails.
  • Seat and back cushions for pressure support.
  • Products to help your memory and thinking.
  • Devices to help you get dressed like zipper pullers and buttoning devices.
  • Speciality clothing and shoes.
  • Incontinence alarms and personal alarms.
  • Items to help you eat like modified cutlery.
Home modifications

Home modifications are changes to your home to make it safer and more accessible. These include things like installing grab rails, modified door locks, and ramps to the entrance of your home and other doors. Home modifications are available even if you are renting.

Our Allied Health team can help you by:

  • Recommending suitable home modifications and equipment to help you live safely at home.
  • Setting up your modifications or equipment and provide advice and support on how to use them.
  • Organising repair to the equipment or modifications if needed. 
Availability

We provide equipment and home modifications in all states of Australia, including the Australian Capital Territory, through the Support at Home AT-HM Scheme.

Eligibility

You need to be eligible for the Support at Home program to receive the Assistive Technology and Home Modifications (AT-HM) scheme funding.

More complex items require a qualified health professional to conduct an assessment. This includes home modifications. If this applies to you, please ask our care team to arrange an assessment for you or contact us on 1800 650 803.

Costs

There are three levels of funding (called tiers) you may be eligible for:

  • Low tier: $500
  • Mid tier: $2,000
  • High tier: $15,000.

You have up to 12 months to use your funding. Additional Assistive Technology funding may be available following an assessment, but home modification funding is capped at $15,000 over your lifetime.

You may need to financially contribute towards the costs of your equipment or home modifications. Services Australia will determine how much you need to pay. For clinical services such as Allied Health, you do not need to pay a contribution, these services are provided at no cost to you. See more information on Support at Home contributions.

Health professionals can refer patients for government funded aged care services by using the My Aged Care online referral tool, or by calling 1800 200 422. You can also make an e-referral from your practice management system.
