Join Silverchain’s panel of aged care experts on 5 August 2026, to learn more about the End of Life Pathway, available under the Australian Government’s Support at Home aged care program.

Using a range of case studies, our team will explain the End of Life Pathway, including eligibility requirements, services inclusions, and how the pathway complements existing generalist and specialist Palliative Care services.

You can watch it live on Wednesday 5 August 2026 at 6pm to 7pm AWST/ 8pm to 9pm AEST or stream the recording afterwards on demand.

The session is eligible for one educational activity CPD hour.

Learning outcomes

  • Define the eligibility requirements for the End of Life Pathway.
  • Outline the assessment process for the End of Life Pathway.
  • Describe the services that can be provided through the End of Life Pathway, and how these differ from Specialist Palliative Care services.
  • Identify services and resources provided by Silverchain to support Palliative Care and End of Life Care patients.

Ronelle Hutchinson PHD

Dr Hutchinson has worked in public policy within and for government, health peak organisations and health providers for over 20 years. She currently leads Silverchain’s federal government relations, and serves on government advisory groups supporting the development of the new aged home care system and strengthened clinical care standards.

Dr O’Donnell-Taylor is a General Practitioner with a special interest in the areas of Women's and Children's health. She is a passionate advocate for the role of Primary Care, quality General Practice training and the welfare of trainees, and system improvement and reform.

Cassie leads a large team of health and aged care professionals to develop and implement innovative care plans, ensure compliance with clinical guidelines and regulatory standards, and monitor patient outcomes.
