Chronic Obstructive Pulmonary Disease (COPD) Community Support Program

This program is a non-urgent clinical care coordination service for patients with COPD that have been discharged from a public hospital following a COPD-related admission. 

Referral criteria

Eligible:

  • Confirmed diagnosis of COPD; and
  • Public hospital inpatient admission related to COPD; and
  • Aged 18 years or over.

Not eligible:

  • Living in a Residential Aged Care Facility
  • Oxygen-dependent

No cost to client.

This COPD Community Support Program is delivered in collaboration between Silverchain and Asthma WA, funded by the WA Primary Health Alliance (WAPHA).


How to refer

Referrals are accepted from public hospitals.  

  1. Check referral eligibility criteria.
  2. To refer, fill in the COPD referral form;

  4. For inquiries, please call 1300 300 122 and ask to speak to our Respiratory Liaison Nurse or Clinical Nurse Manager Respiratory, available between Monday to Friday from 8.00am to 4.00pm.



Domiciliary Oxygen

The Domiciliary Oxygen and Respiratory Service is a specialised service for clients with a respiratory condition needing oxygen therapy at home.

Referral criteria

Eligible:

  • Aged 18 years or over; and
  • Medicare card holder; and
  • Living at home, hostel or independent living facility; and
  • Fulfils the WA Department of Health prescribing guidelines (in the Domiciliary Oxygen referral form).

Not eligible:

  • Living in a Residential Aged Care Facility
  • Current smoker
  • Oxygen referrals for cluster headaches do not qualify
  • DVA Gold Card holders

How to refer

Clients requiring oxygen at home requires referral from a Respiratory Specialist (or other referrers in accordance with the Domiciliary Oxygen referral form).

  4. For any referral enquiries, please call 1300 300 122 and ask to speak to the Clinical Nurse Manager Respiratory, available Monday to Friday from 8.30am to 4.30pm

For newly referred clients, referral must be received prior to 12.00pm on weekdays for commencement of oxygen therapy the next working day.  If you require urgent commencement of oxygen, please call our Ambulatory Liaison Nurse on 1300 300 122.

Please note: If the person requiring oxygen is DVA, please fill in the Application for Home Medical Oxygen Therapy and/or Respiratory Home Therapy Appliances form.
