Supplier Registration

We have strong relationships with our suppliers who support the delivery of our services across Australia.

We extend an invitation for you to complete the registration form as the initial step in the onboarding process.

Should your application advance, we will contact you to undertake the second stage of compliance and the third stage of draft agreement review.

Services cannot commence until the completion of Silverchain's onboarding process, which includes the execution of a fully signed contract.

Payments for services are not backdated, and invoices to Silverchain are permissible only for service dates in line with the agreement's commencement date.

Public Liability insurance in the amount of not less than $10 million dollars in respect of any one Claim and unlimited in the aggregate. Professional indemnity insurance for at least the sum of $10,000,000 (large business) $5,000,000 (small business) for any one occurrence, to be maintained for 6 years after the last Service is provided. Cyber Security insurance for at least the sum of $2,000,000 for any one occurrence. (Large Business) Cyber Security insurance for at least the sum of $1,000,000 for any one occurrence. (Small Business) Motor vehicle insurance for all vehicles used by You or Your Personnel to transport Clients for an amount not less than $20 million for any one occurrence and unlimited in the aggregate. Medical Malpractice insurance for service providing Medical, Paramedical or Nursing Services for at least the sum of $20,000,000 for any one occurrence, to be maintained for 6 years after the last Service is provided. Any other insurance which may be required by applicable Laws, including Workers Compensation insurance and any other insurance for or with respect to Your Personnel, third parties or the Services.

Notes:

* Motor Vehicle insurance is only required if you transport clients in company vehicles

*Service Providers who provide allied health, personal care, social support, respite care, and nursing services are required to maintain cyber security

*Small business Annual turnover: ≤$10 million

*Large business Annual turnover: >$10million

*Please note that if your supplier form registration is approved, you will be required to upload all insurances set by Silverchain to our compliance portal, "Zipline", to which you will be invited during stage 2.

As an authorised representative of the registering organisation, I declare: I understand that, if engaged by Silverchain, my organisation will be considered an Associated Provider under the Aged Care Act 2024 (Cth) . The organisation has a documented system in place to manage and control work health and safety hazards and risks.The organisation has an incident reporting process that ensures incidents are reported and managed promptly. The organisation complies with applicable laws, regulations and standards governing aged care, health care and the services that my organisation provides. The organisation has policies and procedures to ensure that National Police Checks and NDIS worker screening clearances are up to date and compliant for all personnel. The organisation’s personnel have appropriate licenses, qualifications, experience and competencies to provide the services. All personnel who provide personal care, social and/or respite care on behalf of the organisation have Certificate III in Aged Community Care (Ageing). The organisation pays its personnel at or above minimum wage or award rates o The organisation has taken reasonable steps (including reasonable enquiries) to investigate its labour practices and supply chain, and those of its subcontractors, in order to reasonably ensure there is no modern slavery (as defined in the Modern Slavery Act 2018 (Cth) in its structure, operations or supply chain and those of its subcontractors. Any information provided in this registration form is true and accurate, and I will notify Silverchain immediately in the event that any information provided becomes untrue or inaccurate.
