  • Summarise the changes to their home aged care system under the Support at Home program.
  • Define the eligibility requirements for the End of Life Pathway.
  • Outline the assessment process for the End of Life Pathway.
  • Describe the services that can be provided through the End of Life Pathway, and how these differ from Specialist Palliative Care services.
  • Identify services and resources provided by Silverchain to support Palliative Care and End of Life Care patients.

Funding for older Australians with low to high needs to access services to stay healthy and connected at home.

Health professionals can refer patients for government funded aged care by contacting My Aged Care, or by calling 1800 200 422. You can also make an e-referral from your practice management system.

For private services, patients do not require a referral or assessment and can contact us directly. However, you can contact us on a patient’s behalf by calling 1300 300 122.
