At Silverchain, we offer a free Chronic Care Coordination Service to help you manage health conditions such as asthma, cancer, diabetes, heart or lung disease and arthritis in between visits to your GP or health care providers.
How we help:
Working closely with you and your health care providers, our team of nurses and allied health professionals to provide support depending upon your health and wellbeing needs. Our service can include:
- Booking appointments for other services and programs and assist with arranging transport to attend them.
- Finding other solutions when you face long wait times for the services you need.
- Explaining medical language and test results to you.
- Helping you to obtain new or more referrals.
- Assistance with filling prescriptions.
- Setting everything up so you can maintain your health and work towards your health and lifestyle goals.
