We provide a range of in home aged care services that can be funded by the Commonwealth Home Support Program (CHSP) so you can remain living independently at home.
The Commonwealth Home Support Program (CHSP) is an Australian Government funded program for basic in home aged care services.
It is designed for older Australians who need a little extra support to stay safe and independent at home. The program is often recommended by My Aged Care as the first step in your care journey, if you need only one or two services.
At Silverchain, we can provide a large range of services funded by CHSP. We are an accredited aged care provider, which means that the services we provide are expected to meet the same Australian Government safety and quality standards you would expect from an aged care facility.

Eligibility
To be eligible for CHSP funding you will also need to be aged 65 years and over, an Aboriginal or Torres Strait Islander aged over 50 years old or are at risk of experiencing homelessness and are over 50 years old.
You will need to contact My Aged Care (MAC) to request an assessment for this program, or you can ask your GP to refer you to MAC for services.
If you need care while you are waiting for your assessment or funding, you can also self fund any of our aged care services privately. Learn more about our private care.
Do you need more aged care services?
If you already have CHSP funding and need more services, or higher levels of care that your funding can cover, you may be eligible for the Support at Home program.
Under the Support at Home program you will have more flexibility, and can access a wider range of complex services including more complex health care. You will also have a dedicated Care Partner to help you organise your care and services to meet your needs.

Health professionals can refer patients for government funded aged care services by using the My Aged Care online referral tool, or by calling 1800 200 422. You can also make an e-referral from your practice management system.
For private services, patients do not require a referral or assessment and can contact us directly. However, you can contact us on a patient’s behalf by calling 1300 300 122.
