Co-designing home care solutions at GP24
Silverchain recently attended the RACGP GP24 Conference on 21 – 23 November at the Perth Convention Centre. This highly anticipated event brought together General Practitioners (GPs) across the country to exchange knowledge, advancements in care and insights.
Silverchain’s Executive Medical Director East Coast Dr Helena Williams presented on the future of in home care during the Breakfast Symposium. Her presentation ‘Enhancing patient experience and care through a co-design approach to integration: the GP as consultant in primary care’ took GPs through future co-design solutions for patients within our home care system.
As one of Australia’s leading in home care specialists, Silverchain provides an interface between the primary and acute health care sectors. Dr Helena explored how collaboration with us can achieve great outcomes for patients and communities. This included:
- increasing patient choice by enabling the provision of care at home
- avoiding hospital presentations
- improving the patient experience
- improving safety, quality and clinical outcomes.
Dr Helena discussed how our comprehensive range of in home services allow us to provide clinical care such as wound management, specialist and generalist palliative care, and complex chronic disease support.
An integrated approach ensures a smooth transition between providers and greater consistency of care, as well as reducing the burden on GPs dealing with multiple care providers and the disconnection with their patient’s care team.
GPs and their patients can leverage our specialised services such as wound or palliative care, in a model of care that supports GPs as Consultants. Providing coordinated care instead of fragmented services from multiple providers.
Using models of care and case studies, Dr Helena demonstrated the clinical and socioeconomic value of home based primary and community care.
She also talked to the advantages of shared long term relationships with patients, the use of technology to support improved communication and care coordination, and our vision for delivery of high performing interdisciplinary teams.
Dr Helena’s presentation was highly engaging and encouraged GPs to take a fresh look at the potential opportunities of collaborative models of care with Silverchain.
