WA operating hours and services will change over the holiday season. Here’s what you need to know.
The following services operate as usual:
- Care Plan Support and Coordination Service
- Primary Care at Home
- Health Navigator
- Post-Acute Care
- Community Specialist Palliative Care Service
- Continence Management and Advice Service: Please note we are currently experiencing a very high level of demand, and as a result there is a waitlist. Confirmation letters will be issued to clients and referrers upon receipt of referral.
- Hospital Discharge Support: Please note this service is unavailable on public holidays. Initial visits for accepted referrals will be completed on the next working day or other agreed day.
- Complex Nursing and Commonwealth Home Support Programme Nursing: Please note that first visits are unavailable on public holidays.

Clinical Services (regional)
First visits are unavailable on weekends or public holidays. For referrals received on Monday 23 December 2024 the first visit post-referral will depend on available resources.
Hospital at the Home and Priority Response Assessment
We accept referrals over this season with the normal cut off time of midday. If the first visit falls on the 25 to 27 December 2024 or 2 January 2025, referrers are asked to provide prescribed medications to cover these days.
In Home Aged Care Services
Services continue to accept referrals and arrange services as soon as possible.
- Commonwealth Home Support Programme: Services for existing clients operate per individual arrangements over the holiday season. First visits accepted immediately prior to and during the public holiday season will occur after Thursday 2 January 2025.
- Allied Health Services: Services are delivered according to priority over the holiday season.
- Home Care Package Nursing: Services operate as usual. Requests for new nursing services will be delivered according to priority and capacity over the holiday season.
Oxygen & Respiratory Service
- Service delivery dates depend on when referrals are received:
- Before 2pm Monday 23 December 2024: services delivered on Tuesday 24 December 2024.
- After 2pm Monday 23 December 2024 and before 2pm Friday 27 December 2024: services delivered on Monday 30 December 2024.
- Before 2pm Monday 30 December 2024: services delivered on Tuesday 31 December 2024.
- After 2pm Monday 30 December 2024 and before 2pm Tuesday 31 December 2024: services delivered on Thursday 2 January 2025.
- On Wednesday 1 January 2025: services delivered on Friday 3 January 2025.

The clinic is closed from 1:30pm Friday 20 December 2024 until 8:30am Friday 3 January 2025.
