The Transition Care Program (TCP) provides short term funding for services designed to help you get better after a hospital stay.

To be eligible for TCP you will need to be aged 65 and over, or aged 50 and over for Aboriginal and Torres Strait Islander people. You must also be a current hospital patient ready for discharge and have a need for short term support. Services can be provided for up to 12 weeks in your home or in the community.

At Silverchain, we provide TCP services to clients in the Great Southern in Western Australia (WA), and in the Sunshine Coast Hospital and Health Service catchment area of Queensland (QLD). These include:

  • Personal Care
    Support with daily tasks like showering, getting dressed, and taking your medications.
  • Allied Health Services

    Support to restore and maintain your mobility and improve your independence at home.
  • Nursing Services
    Tailored care to help manage and support your health care needs. 

Availability varies by location, so contact us to see what is available in your area.

To access TCP support, you will need to be assessed while you’re in hospital. You can ask your hospital staff to organise this, or you can schedule an aged care assessment through My Aged Care (MAC).
