In partnership with Asthma WA, we provide nursing care to help people manage their Chronic Obstructive Pulmonary Disease (COPD) condition on an ongoing basis.
COPD is an ongoing lung condition that causes coughing and makes it harder to breathe over time. It is a broad term used for a group of conditions like emphysema and chronic bronchitis, and asthma.
Without the right care, you may have exacerbations or flare ups, which is when your symptoms get worse suddenly, this means you may need to go to hospital. Through our program, you can learn to manage your COPD condition at home and avoid going to hospital.
Our Respiratory Nurse will visit you at home or call you to ask about your current health, your knowledge of your condition, past flare ups, and symptoms.
Based on this information you provide, we can provide tailored advice and interventions to help you manage your condition, including help in monitoring your symptoms and reducing flare ups. We will also talk you through your action plan, so you know what to do at each stage.
You can then receive ongoing support from Asthma WA for up to six months after leaving hospital.

Our COPD program is available in Perth, Western Australia (WA) only.
Private Care
Support for people with COPD is also available as a fee for service in Perth and some regional areas of WA. Availability will vary by location, please contact us to find out what is available in your area.
People aged 18 and older with confirmed COPD diagnosis and have been admitted to a public hospital due to their condition may be eligible for this program.
You will need a referral from your hospital clinician to access this program. All people 18 and over are eligible for private services.

This program is funded by the WA Primary Health Alliance (WAPHA) and is free for clients with a referral.
