In partnership with Asthma WA, we provide nursing care to help people manage their Chronic Obstructive Pulmonary Disease (COPD) condition on an ongoing basis.

Chronic Obstructive Pulmonary Disease (COPD) is an ongoing lung condition that causes coughing and makes it harder to breathe over time. It is a broad term used for a group of conditions like emphysema and chronic bronchitis, and asthma.

Without the right care, you may have exacerbations or flare ups, which is when your symptoms get worse suddenly, this means you may need to go to hospital. Through our program, you can learn to manage your (COPD) condition at home and avoid going to hospital.

Our Respiratory Nurse will visit you at home or call you to ask about your current health, your knowledge of your condition, past flare ups, and symptoms.

Based on this information you provide, we can provide tailored advice and interventions to help you manage your condition, including help in monitoring your symptoms and reducing flare ups. We will also talk you through your action plan, so you know what to do at each stage.

You can then receive ongoing support from Asthma Western Australia (WA) for up to six months after leaving hospital.

Find out how to get started, so you can receive care at home, call our Contact Centre staff or complete our contact form today.
