Our HIV Enhanced Primary Care Coordination Program provides comprehensive nursing services, including case management to people with HIV living in South Australia. 

We can support you at home, at a location of your choice, or in one of our nursing clinics to understand your diagnosis and treatment options.

Our skilled nurses will complete an assessment and develop a care plan based on your needs, which includes strategies to maintain your health and access care services. 

Our care team can also link you to other services, such as:

  • counselling
  • dentistry
  • sexual health services
  • drug and alcohol services
  • GP and peer support
  • aged care services.

Our HIV program is available in South Australia (SA). Program support outside metropolitan Adelaide is provided through telehealth.

This program is not currently available in other states.



Eligibility

People with a new or existing HIV diagnosis who is new to treatment, or those with complex needs, may be eligible for this program.





Costs

This program is funded by SA Health and is free for clients with a referral.
