There are a number of fees and charges included in your Home Care Package, in addition to the cost of your services. These include Care Management, Package Management and Supplement Costs.

Care Management

Care management is an important service that includes coordinating care and services that help you deliver on the goals you identified in your Care Plan. Every Home Care Package requires some level of care management.

Your Coordinator or Manager is your point of contact with us, and with your permission they can work with your carers/family, and other health professionals like your doctor to ensure that all of your needs are met. 

They will work with you to develop a care plan and review it regularly. This means we can make changes to your care over time, aligned to your goals and preferences, especially as your needs change.

At Silverchain we generally offer our clients provider managed care instead of self managed care. If you would like assistance with managing your care arrangements and budget, and proactive reviews to determine if the services you receive meet your needs, then provider managed care is the best option for you.

Package Management

Package management is a mandatory service as it covers the costs of administering your package funds. 

This includes the administrative work required to manage your funds, prepare your monthly statements, as well as the costs associated with coordinating services. It also includes quality assurance and compliance activities.

As a not for profit organisation, our priority is your care. We aim to keep our package management costs to a minimum, so the majority of your package is spent on you and your needs.

Basic Daily Fee

Providers can choose to charge this fee which increases the funds available in your package. This fee is capped at 15.68% to 17.50% of the single basic age pension, depending on your package level. 

We do not charge a Basic Daily Fee unless otherwise agreed with you in your Home Care Package Agreement.

Income Tested fees

The income tested fee is set by the government and something you may pay based on your assessable income. Your assessable income does not include the value of your home or other assets.

Travel Costs

We do not charge travel costs to visit you. This means you can use more of our funding on the high quality, essential services you need.

Supplements

If you receive a package, you may be eligible for additional supplements. This can be for specific care needs, such as dementia, cognition support, and oxygen or enteral feeding, or a Veterans’ supplement. Ask your care coordinator about supplements when you meet with them.
