Silverchain partnered with clinicians, referrers, and medical experts to implement a series of professional development sessions to support integrated patient care across hospital, home, and community.
Research has shown that better continuity of care across acute and primary care can significantly improve patient outcomes. This leads to less hospitalisations, lower mortality, and higher levels of patient and carer satisfaction.
The Silver Learnings series, launched in 2023-24, is one of the ways Silverchain is partnering with clinicians and referrers to help patients receive the quality care they need.
Designed for doctors, practice managers, and specialists, these sessions cover a variety of topics such as chronic disease and palliative care. They are presented by health industry leaders and Silverchain experts and offer continuous professional development (CPD) hours, practical real-world case studies, and opportunities to engage with industry leaders and peers.
In 2023-24, we held successful Silver Learnings sessions on chronic disease management, home hospital care, wound care and palliative care in Western Australia.
Our wound management session saw our world leading wound care practitioners share their knowledge and support attendees. Booked out due to high interest, the session covered wound assessments and identification of simple to complex wounds, management strategies, and an overview of the health services, resources, and referral pathways available. Silver Learnings will be expanded nationally next year.
Silverchain is one of a few Australian home care providers accredited in both the national health care and aged care standards.
This means the services we provide in your home are expected to meet the same safety and quality standards you would expect from a hospital or aged care facility.
In 2023-24 we successfully achieved accreditation in South Australia against the Aged Care Quality Standards. Bunbury, Western Australia was also assessed and achieved accreditation. We are preparing for national health care accreditation in late 2024.
We also launched a new partnership with Australian Clinical Labs (ACL) to offer priority in home pathology tests to eligible Western Australian clients.
The priority at-home service will initially be provided to Silverchain’s eligible clients in Western Australia, ahead of a national rollout of the service for clients in South Australia, New South Wales, Victoria and Queensland.
The collaboration between ACL and Silverchain will mean improved access and efficiency that will ultimately improve health outcomes for patients.
In 2024, we were honoured to be recognised as a Gold Winner in the highly competitive category of Home Care Services in Western Australia, as judged by the Australian public in the Reader's Digest 2024 Quality Service Award Survey. This is the third year in a row we have won this award for our home care services in Western Australia.
We were also voted as the most trusted brand in home care in Western Australia for the third year running, and South Australia in 2023. It is a reflection that our name is synonymous with the provision of safe, personal, connected and effective Best Care.
