We’re looking for Case Coordinators to join our Sunshine Coast leadership team. In this role you will be responsible for the allocation of client visits including new referrals, coordination of resources, clinical care delivery, and monitoring the delivery of direct care, and coordination of team meetings.   

You’ll also provide leadership and supervision to promote ongoing development of our direct care teams ensuring compliance with Silverchain policies and procedures enabling the delivery of high-quality care.  

You will work closely with the Clinical Nurse Managers to ensure the delivery of clinical services for clients requiring a higher level of care and provide support for nurses, care workers, student placements and volunteers. 

You will also have a current nursing registration with the Australian Health Practitioner Regulation Agency with demonstrated high level clinical skills and understanding of care delivery within a community environment.  

We’re looking for people who can working across seven days between 6.00am – 8.30pm

Pay rate: $48.30 to $53.62 per hour.

We’re looking for Assistants in Nursing to join our team and help provide services to more than 115,000 Australians each year. Our AINs deliver exceptional care and support to our clients, enhancing and encouraging their ability to participate in their own care and maintain their independence.  

In this role you will provide support with skin care, toileting, general hygiene, grooming and dressing, mobility, meal preparation, and associated personal care activities. Support may also be required with general housekeeping, social support, and transport. 

To succeed in this role you will have completed a Certificate III in Aged Care, Individual Support, or have commenced studying an equivalent qualification.  Desirable: experience in delivering moderate or complex care, preferably within the community; strong interpersonal skills; a current driver’s license, and a commitment to delivering the highest standard of care.  

We’re looking for people who can working across five day work week between 7.30am – 4.00pm  

Pay rate: $28.05 to $29.24 per hour.

We are seeking Registered Nurses (RN) and have a range of casual and permanent part- or full-time roles available across the Sunshine Coast.  

Join our growing team and provide best care for a variety of services to our clients in their homes, including Hospital in the Home, Post Acute Care, Transition Care, and Palliative Care.

You will also conduct clinical assessments, implement and manage care plans, help our clients with catheter care and IV administration, and provide wound care in the home.  

For this role you will need a current driver's license and broad experience in a general nursing role and current APRHA registration. Experience working in community care will be highly regarded.
