Addressing the challenges of an ageing population, the increasing burden of chronic disease and rising cost pressures requires innovative thinking, shared expertise, resources and capacity that no single organisation can support alone.
To offer our expertise and support to more Australians, we grew our network of partnerships during 2021-22. We partnered with South Eastern NSW Primary Health Network to deliver Coordinare. The new care navigation service supports one of the largest rural and regional populations in NSW. Geographically, this stretches from is most southern point in Helensburgh in the north to the Victorian border, north and inland to Cooma/Monaro, Queanbeyan, Yass and Goulburn.
Through Coordinare, our care coordinators partner with primary health care providers to support their higher needs patients who need extra help to navigate the health system to better manage their chronic health conditions. This includes assisting patients in making their health care appointments, arranging their transport, and liaising with their network of care providers.
In 2021-22, Silverchain also partnered with Ingenia Communities as its preferred provider of home care packages. Ingenia is a leading seniors’ living operator in Australia. Our partnership with Ingenia will support more people to live independently at home.
The partnership with Ingenia was born out of shared values between the two organisations – that everyone has the right to live a healthy and independent life in their own home, receiving high quality care that meets their needs, preferences and personal values.
Silverchain ran seven successful pilots at Ingenia’s lifestyle villages in 2021-22 – three in regional Western Australia, two in regional Victoria and two in metropolitan Melbourne.
We also continued our successful partnerships with the WA Health, WA Country Health Service, SA Health, Sunshine Coast Hospital and Health Service, Cairns and Hinterland Hospital and Health Service, Royal Flying Doctor’s Service, and Western Sydney Local Health District.
