Fee-for-service (FFS)

If a person does not meet the eligibility criteria for government-funded programs, a range of services can be provided on a fee-for-service basis:

  • Nursing
  • Occupational Therapy
  • Physiotherapy
  • Podiatry (metro only)
  • Personal Care
  • Domestic Assistance

Referral criteria

Eligible:

  • Age 18 years or over.
  • Clients do not need to be an Australian citizen or permanent resident. 
  • Clients living in their own home, retirement villages or independent living units.
  • All functional levels.

How to refer
    1. Call 1300 300 122 for current availability and to obtain price of services.
    2. The Silverchain Contact Centre representative will send you a Payment of Services Agreement form to complete.
    3. Services can commence when the payment of services has been agreed.
