A Home Care Package (HCP) is a funding package from the Australian Government designed to subsidise the cost of in home aged care for older Australians.
A HCP is often the next step on from receiving basic home care services through the Commonwealth Home Support Programme (CHSP). This is because it provides funding for you to receive a higher level of supports and care.
Depending on your needs there are four levels of care that you can receive funding for. These range from a Level 1, which may include home and garden maintenance, to Level 4 for complex care like nursing and allied health care.

Our HCP services are available in Perth and regional areas in Western Australia (WA), in Adelaide in South Australia (SA), and greater Melbourne and Golden Plains and Moorabool areas in Victoria (VIC). We also provide a specialised Dementia Wellbeing Service (DWS) in WA through HCP.

Managing your package
With a HCP, you can choose to have your package managed by your provider (provider managed) or by yourself (self managed). Self management allows you to organise your own supports and manage your funding and care plan.
At Silverchain we generally offer provider managed care to our clients. If you’ve chosen us to manage your care, you will have a dedicated Care Coordinator or Case Manager to help you do this.
