We can help you understand the fees and charges that may apply for our services.

At Silverchain, we partner with state and federal governments and local health networks to provide a range of in home health and aged care services at no or low cost to you. 

Most of our services are funded by the government. This means if you’re eligible the government will subsidise all or most of the cost of your services that you receive. For services that are partly government funded, you will need to contribute to the cost of your services, if you can afford to do so.

Your contribution will vary based on the type of government funding you have, your financial situation, and the services you receive. We will explain the costs that apply to you and help you understand your options if you cannot afford to pay. This includes who to contact to answer any questions or concerns.

Our fees ensure that we can continue to provide you with the best care.

Aged care service fees

The Australian Government expects all people to contribute towards the cost of their care, if they can afford to do so. How much you need to contribute is determined by Services Australia and is based on an Income and Means Assessment.

Commonwealth Home Support Programme (CHSP)

We provide CHSP services in Queensland (QLD), New South Wales (NSW), Australian Capital Territory (ACT) Victoria (VIC), Tasmania (TAS), South Australia (SA) and Western Australia (WA). Our current price lists are below: 

If you are experiencing financial hardship you may apply to us, to waive your financial contribution towards the cost of your CHSP services. You can download a copy of our Fee Waiver Form and send it in to us.

Home Care Package (HCP) and Support at Home (SAH)

If you are eligible, the Australian Government will subsidise your in home aged care, and will provide you with a Home Care Package (until 1 November 2025) or Support at Home funding (after 1 November 2025).  Your funding will be based on your My Aged Care Support Plan that outlines funding for the supports you need.

Contributions towards your care

The Australian Government asks that you financially contribute to the cost of your Support at Home services. if you can afford to. This is called your Contribution Rate. 

Your Contribution Rate funds some of the service costs and is part of your total Support at Home ongoing budget. Your Contribution Rate is determined by Services Australia.

If you have not competed an Income and Means Assessment, your contributions under the Support at Home program will be set to the default level which is 80% of the cost of Everyday Living supports, and 50% on Independence supports.

Funded health care services

Most of our health care programs are funded by state governments and are free for eligible clients with a referral. 

This includes programs such as Specialist Palliative Care, Home Hospital and Post-Acute Care, Continence Management and Advice Service (CMAS). Refer to us

Health professionals can refer patients for government funded aged care by contacting My Aged Care, or by calling 1800 200 422. You can also make an e-referral from your practice management system.

For private services, patients do not require a referral or assessment and can contact us directly. However, you can contact us on a patient’s behalf by calling 1300 300 122.
